
The NSW Agency for Clinical Innovation (ACI) Transition Program assists young people with chronic and complex health conditions as they move from children's to adult health services.
There are three area coordinators for NSW. Kaleidoscope is serviced by Northern area.
Transition should be well planned and begin early to ensure an uninterrupted, coordinated, and smooth transition from child centred to adult oriented health care.

The ACI Transition Coordinators work closely with medical and nursing staff to make sure all young people have a transition process in place.
We can:
Referral to our service can be made by the young person, parent / carer or other family member, paediatric or adult clinical team, nursing or allied health team (physiotherapist, occupational therapist and nutritionist).
Clinicians can complete the referral form (doc 69K) and fax / e-mail to the Transition Co-ordinators or we can be contacted at our office by phone or email as listed below.
