| Referral Needed: | Written referral required from a doctor (GP or specialist). Referrals need to be faxed to the Referral and Information Centre on 4925 7950 |
| Eligibility criteria: | Pension card or health care card AND one of the following: a) Foot pain and/or b) a medical condition that puts foot health at risk (i.e. diabetes, neuromuscular disorders, etc). |
